Coverage B (80% coverage): Minor restorative; fillings; amalgams; and resin treatments. Non-surgical periodontics including treatment for the diseases of the gums. Coverage C (50% coverage): Major restorative; cast restorations; crowns; onlays; and other ceramic restorations. Other items include prosthodontics (including bridges, partial dentures, and complete dentures). Also provides coverage for surgical periodontics necessary for the treatment of diseases of the gums. Coverage D (50% coverage): Orthodontics necessary for proper alignment of teeth for covered dependent children under the age of 19. Sometimes available for adults.
